SYLLABUS

CONGRATULATIONS: Welcome to CSM 750 Clinical Practicum in the Case Management Master’s Degree program. Achieving a Maters degree in Managed Care is a very good career decision. Today’s employers are searching for professionals who are degreed in this specialty. Managed care is an exciting field with great demand for skilled professionals. It’s a field that can provide you with challenges and professional growth for years to come. This advanced course will teach you to use appropriate medical and social resources, comply with HIPPA, communicate effectively with payers, and learn methods to monitor outcomes. I look forward to working with you as you progress through the program.

WHAT IS A CASE MANAGER? Well, you might think of a case manager as a conductor of an orchestra of talented musicians (medical professionals) playing for an audience of one (the patient). This conductor uses knowledge and skills to manage members of the orchestra, keep the tempo correct, and see that everyone plays on the same page of music. But what if the conductor hears discordant notes when there should be a melody? If the various instruments aren’t mixing well, the conductor identifies that section of the orchestra, and brings in a specialist to make the music work. If sour notes are still heard, the conductor finds and corrects the problem before it gets worse. By bringing the many musicians together harmoniously, the orchestra works for the benefit of the audience (the patient). Keeping it all cost-effective, of course.
In other words, the case manager—you—coordinates the various elements of medical care to return the patient to optimum health in a cost-effective delivery system.

This course will allow you to use the knowledge gained from your other courses to move through a case from its beginning to its conclusion. You will draw upon your knowledge of texts, community and Internet resources to solve problems that will occur along the course of the clinical practicum. The course will allow you to use your analytical and problem-solving skills, and you are encouraged to be professionally creative in responding to assignments.

TIME FRAME: This is an eight(8)-semester hour course. This course is allotted fifteen (15) weeks of time. You must complete all the requirements for the course successfully by the end of the ten-week period. The first day of week one will begin the day that you register for the course, or the day, which you notify me that your textbook(s) have arrived and you are ready to begin your studies. Please be cognizant of the time frame. It is rare that extensions of tie are permitted, unless the student has good justification. Upon successful completion of this course, you will be awarded eight(8) semester hours of credit.

EXAMINATIONS: There is one examination for this course. This final exam is taken by the end of week fifteen of the course. The final exam questions will be taken from your texts.

The exam is programmed and located in the classroom for this course. The examination is “open book” objective type. You will have one (1) day to complete your exam, once you access it from the classroom. To access the exam, you must send me an email and request to have access to the exam. Upon registration, you will receive your email address. I will then program your access in. You will receive an email back from me telling you that you are now authorized to go ahead and to access your exam. To access, you will come into the classroom, click on testing, and click on the exam you are taking. You will need your User ID and PASSWORD to access the exam. The exam will appear on your screen. Once you access the exam, you have 24 hours to submit your answers. The program provides me the exact date and time that you accessed the exam. The program also notifies me of the exact date and time that you submitted your answers. Thus, the program is timing you. When you are ready, go back into the classroom and click in your responses and then click submit. Shortly, you will receive the exam in your email box with your computed score. You will also know what questions, if any, that you missed, and what the correct answer is. I also receive a copy of your exam and your score.

GRADING: The grading scale for this course is as follows:

90-100%    = A
80-89%      = B
70-79%      = C
Below 70% = Fail

ASSIGNMENTS: For specific assignments, please click on the link “Assignments.” There will be ten written assignments, one per week. Each will require 2-3 pages response. You can email your completed assignments to the address provided.

ABOUT THE INSTRUCTOR: My name is Leah Meredith Klein. I graduated with a BSN from Alderson-Broaddus College in Philippi, West Virginia. I later graduated with a Master of Curriculum and Instruction (in the field of nursing education) from the University of Wisconsin-Madison at Madison, Wisconsin. Later, I returned to UW-Madison to complete a Master of Science of Social Work. I have blended the two fields in case management and rehabilitation during my thirty-year experience in the fields. My experience has included teaching, research, case management, and private practice psychotherapy. I am also an avid kayaker and when I’m not at my computer, I’m on the water.

COMMUNICATION: You are encouraged to communicate with me. I am available as a teacher, coach, and mentor to assist you in meeting your goals for this course. Primarily, communication is through email. However, I am also available for conversation by telephone if you would like. Our classroom for this course has a “chat” room. I am also very willing to meet with you one-on-one in the chat room at your request. From time to time, depending on how many students are enrolled in this course at a particular time, we will have some scheduled group chats. You will receive more detailed information at the time such chat sessions are scheduled. Please keep my email address handy so that you can contact me whenever necessary. 2). Identify the role of payers: Medicaid, Medicare, HMOs, PPOs, Private Insurance.
3). Understand Confidentiality of Medical Records, Electronic Submissions and HIPPA.
4). Understand methods of making effective decisions.
5). Understand the skills needed to communicate and persuade effectively with claims adjusters, medical professionals, families and clients.
6). Use appropriate resources in case management.
7). Coordinate the elements of a medical case and learn methods to monitor outcomes.
